This  is an online E log book to discuss our patient's de-identified health data shared after taking his/her/guardian's signed informed consent. 


Here we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those patient's clinical problems with collective current best evidence based inputs. 

This E log book also reflects my patient-centered online learning portfolio and your valuable inputs on the comment box is welcome.


Here is a case i have seen:

A 50YR old male who is a shepherd by occupation came to the opd with c/o low grade fever associated with chills and rigor and generalised weakness  since 7days . 
Nausea since 7days but no vomitings and loose stools. Yellowish discloration of eyes associated with yellowish discolouration of urine since 7days.
Grade 4 shortness of breath since 5days
Facial puffiness since 5days
Pedal edema up to ankle since 5days

No h/o cough,cold,decreased urine output,burning micturation,palpitation,
H/o of walking in muddy water 2mnths ago
He is a known alcoholic since 5-6yrs (last binge 1month ago)

N/k/c/o HTN,DM,Asthma,CAD,TB,epilepsy 

No relevant family history 

GENERAL EXAM:

BMI:
Icterus present.
No signs of pallor,cyanosis,clubbing,lymphadenopathy,dehydration 
Temp : 99.6F  BP:110/80mm hg   PR:80bpm
Spo2:98%@RA. GRBS:98mg/dl

CVS EXAM: s1 s2 heard

RESP EXAM: centrally positioned trachea,BAE +,NVBS Heard

PER ABD: decreased upper abdomen expansion ,tenderness in rt hypochondrium,dilated superficial veins over rt lateral aspect of abd,no palpable mass,normal hernial orificesno free fluid,no organomegaly,Bowel sounds heard

CNS EXAM: NAD












Provisional diagnosis : fever with acute kidney injury,acute liver injury
Hypoalubuminia under evaluation
